Peachtree Travel Clinic is conveniently located in the heart of Buckhead at 35 Collier Road, Suite #640, on the Piedmont Hospital campus as well as there is an office location in Fayetteville at the Piedmont Fayette Hospital located at 1265 Highway 54, Suite 201.
Appointments are available Monday, Tuesday, Thursday, and Friday, 7:30 am to 5:30 pm and Wednesday 8:30 am to 5:30 pm at the Piedmont Hospital/ Buckhead office and by appointment at the Fayetteville office. Please call the travel clinic at (404) 351-2572 or email at nurse@peachtreetravelclinic.com to schedule an appointment. If you are new to the clinic and will be visiting for the first time, then please complete the New Patient Form (Adobe pdf*) or if you have been to the clinic previouslt, then please complete the Established Patient Form (Adobe pdf*). You can fax the completed form to us at (404) 835-8513 or email it to nurse@peachtreetravelclinic.com. Please note that we cannot assure confidentiality of any emailed documents or information.
