Appointments
Peachtree Travel Clinic is conveniently located in the heart of Buckhead at 275 Collier Road, Suite #450A, on the Piedmont Hospital campus.
Appointments are available Monday through Friday, 8:00 am to 5:00 pm. Please call the travel clinic at (404) 351-2572 or email at nurse@peachtreetravelclinic.com to schedule an appointment. If you are new to the clinic and will be visiting for the first time, then please complete the New Patient Form (Adobe pdf*) or if you have been to the clinic previously, then please complete the Established Patient Form (Adobe pdf*). If you are just requesting vaccinations that are not for travel (such as meningococcal vaccine for school, zoster [shingles] vaccine, etc), then please complete the Vaccination Request Form (Adobe pdf*). You can fax the completed form to us at (404) 835-8513 or email it to nurse@peachtreetravelclinic.com. Please note that we cannot assure confidentiality of any emailed documents or information.
